Media Release for Minors
If you plan on having your photo or video taken for publicity and are under 18 years of age, please complete this release.
Media Release (Class, Performance, or Event)
This release allows the District to use any kind of student content (whether it be a student submission or of a student performance or show) to make and use videos, digital content, photographs, or any other medium without limitation for all promotional materials, in perpetuity. For example, if a student submits a piece of art for a show or short film, this could be reproduced by the District for its promotional materials but equally if there are photographs or recordings of a student show, class, or performance the District could use those if this release has been signed. Anytime the District is going to want to reproduce or use student submissions or capture a student performance, show, etc. for its use, this release should be signed.
Waiver & Release Agreement
This agrement releases the District from liability for personal injuries when a student participates in some kind of physical activity, whether that be a class, performance, or other activity. You will not need this Waiver & Release Agreement every time you need the Media Release (Class, Performance, or Event) – for example, if a student submits a piece online for some event then this Waiver & Release Agreement is not necessary. But if a student is participating in a dance performance that will be recorded and photographed then both releases, Media Release (Class, Performance, or Event) and Waiver & Release Agreement, should be signed – one allowing the photographs or recordings to be used by the District and the other to release the District up to a certain extent (gross negligence isn’t covered) if the student is injured in the performance. The Waiver & Release Agreement would also be used for classes of a physical nature or other activities that may pose a risk. It’s not possible to list out every situation this may be needed as class, activities, and hosted events at the District are numerous and changing but you may lean towards having the Waiver & Release Agreement signed if there is a risk posed by the class or activity.
